Insurance

 

Currently, I am in-network for Aetna and Optum/United Health Care in California and Colorado. I am also an in-network provider for Humana and Kaiser in Colorado. I am not considered in-network in other states. Please contact me for further information. If I am not in-network with your insurance, you are responsible for my full fees. If you are interested in using your out-of-network insurance benefits, I will provide you with a superbill that you can submit to your insurance upon request.  Keep in mind, submitting a superbill to your insurance company requires that I provide a mental health diagnosis and may result in further request for records. When using out-of-network benefits, it is up to you to negotiate and pursue reimbursement. I cannot guarantee that you will be reimbursed for any of my services. Certain services are likely not covered by insurance such as Parent Consultation.

Payment

 

Payment is due at the time of service.  You can pay by credit or debit card.  You will be asked to keep a credit card on file.  

Cancellation Policy

 

A minimum 24 hour notice for cancellation is required.  You will be charged $75 if you have given less than 24 hours notice or you do not show up to the appointment. One late cancellation or no show fee will be waived per year as a courtesy.

Right to Good Faith Estimate

You have the right to receive a “Good Faith Estimate” explaining how much your medical and mental health care will cost. Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the expected charges for medical services, including psychotherapy services. You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency healthcare services, including psychotherapy services. You can ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule a service. If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill. Make sure to save a copy or picture of your Good Faith Estimate.
